When is the best time to start orthodontic treatment for children?

Hello, this is Dr. Jeong-Won Youn from Yonsei JW Dental Clinic in Gangnam, Seoul, Korea.

Many parents of elementary and middle school students are curious about the ideal time to begin orthodontic treatment for their children. Particularly for elementary-aged children, this concern arises because they are still in a phase of active growth, their permanent teeth may not have fully erupted yet, and parents often wonder if their children can endure the treatment process.

In some cases, children may have congenital issues such as missing permanent teeth under their primary teeth or extra teeth that remain impacted (buried within the gums). They may also have jaw growth irregularities, such as a prominent lower jaw (underbite) or an underdeveloped lower jaw (overbite).

While there is no strict age limit for starting orthodontic treatment, children as young as elementary school age can begin standard orthodontics. However, it is crucial to monitor their growth through regular check-ups and determine the appropriate timing for treatment. For growth-based orthodontics, which utilizes the natural growth of the jawbone, treatment should begin during the jaw's growth phase for maximum effectiveness. Therefore, a detailed examination around the ages of 6–7 is essential to evaluate the ideal time to start treatment.

"The most optimal time to begin orthodontic treatment is during the growth phase."

During growth periods, tooth movement is relatively faster and smoother, which shortens the overall treatment time. Additionally, jaw growth abnormalities, such as underbites or overbites, can be corrected to some extent. Early intervention to guide jaw growth can reduce the likelihood of requiring corrective jaw surgery in adulthood.

In childhood and adolescence, growth plate analysis through hand bone X-rays can determine the current growth stage and the amount of remaining growth. This information helps identify the best time for orthodontic treatment and plan the treatment method. At Yonsei Jeongwon Dental Clinic, we perform hand X-rays and utilize the VUNO Med BoneAge system to assess growth plates.

The VUNO Med BoneAge test analyzes hand X-ray images to determine current bone age and predict future height. It boasts approximately 72.5% accuracy, with over a 15% improvement in diagnostic consistency compared to manual analysis by physicians.

  • In cases of skeletal growth imbalances, such as underbites (prognathism)

It is best to begin treatment as soon as an underbite is observed, ideally during the mixed dentition stage (around ages 6–7), when baby teeth are being replaced by permanent teeth. Starting treatment before the onset of the "pubertal growth spurt" can lead to more effective outcomes.

  • In cases of skeletal growth imbalances, such as a receding chin (retrognathia):

Girls typically experience their "pubertal growth spurt" between the ages of 10–12, while boys experience it about two years later, around ages 12–14. This phase is characterized by rapid growth, not only in height but also in the development of facial bones. By taking advantage of this period of accelerated growth, it is possible to effectively address and improve a receding chin.

  • When there are no skeletal issues, but the problem lies in the position and alignment of the teeth

Orthodontic treatment can begin when the baby teeth have fallen out and most of the permanent teeth have erupted, typically around the upper elementary to middle school years.

The more severe the skeletal issues, the sooner orthodontic treatment should begin. The goal of growth-phase orthodontic treatment is to utilize the upcoming pubertal growth spurt to control the growth of the facial bones, reduce skeletal imbalances, and redirect the growth in a more favorable direction.

"However, it is never too late to start orthodontic treatment,
even if it wasn't initiated during the growth phase."

Recently, adult and middle-aged orthodontics have become increasingly common. While the treatment duration may be longer and the risk of dental side effects slightly higher compared to starting during the growth phase, the benefits of achieving a healthy bite through orthodontic treatment are significant. It can lead to healthier teeth and gums overall.

At Yonsei JW Dental Clinic, the cost of orthodontic treatment includes fees for detailed diagnostics, monthly treatment visits, and other related expenses (excluding retainer costs), so you don't need to worry about unexpected costs. Additionally, since general dental services are available on-site, all necessary treatments, such as extractions or cavity treatments, can be done in one place. To reduce the financial burden of paying a large sum at once, we also offer a payment installment plan, making the treatment even more attractive.

Dr. Jeong-Won Youn, the head of the orthodontic department, has over 15 years of clinical experience, specializing exclusively in orthodontics and handling numerous complex cases. As a result, he is highly skilled in effectively treating a wide range of orthodontic issues, including malocclusion, protruding teeth, and underbites.
